  • If you are at risk of physical harm but do not work in law enforcement, submit a copy of a court protective order, a copy of a police report or similar documentation such as a letter from a shelter administrator or a health care professional

  1. Visit the optout form on the LexisNexis website at https://optout.lexisnexis.com/oo.jsp
  2. Enter your full name, date of birth and social security number along with your address, email and phone number.
  3. Select a reason for the request to remove your personal record.  If you choose an option other than “I do not want my information publicly available via LexisNexis products“, you will be required to submit documentation either attached to the online form or via mail or email as mentioned below.  If you select the option that mentions that says “I do not want my information publicly available via LexisNexis products“, you will only be requesting to be removed from their publicly accessible sites such as KnowX and PeopleWise.
  4. Submit the form with the attached supporting documents (if required)
